29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Room 2/ North End -  Employees are exposed to amputation hazards from the lack of guarding on the Shanklin Wrapper machine (Model F-1-M), on or about 8/1/13.   b) Warehouse / Aisle 2: Employees are exposed to amputation hazards from the lack of guarding on the Shanklin Wrapper Machine (Model F-1-M), on or about 8/1/13.  c) Warehouse / Aisle 3: Employees are exposed to amputation hazards from the lack of guarding on the Shanklin Wrapper Machine (Model F-1-M), on or about 8/1/13.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
