29 CFR 1910.23(c)(2): Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent) and toeboards: (Note: Toeboards are only required where tools, machine parts, or materials are likely to be used on the runway.) (a) On or about July 15, 2013, the runway platform to the shredder/hammermill top was used for access to the top and there was no midrail on either side of the runway.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) The outlets in the maintenance garage along the south wall (closest to the offices) had open grounds. (b) The metal enclosure for the power cabinet in the control tower for the shredder/hammermill was not properly grounded.
