29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports.  Duhon Blvd., Amelia, LA:  On or about July 24, 2013, the employer did not ensure that the working platform of the scaffold was fully planked.  Employee(s) were working from a plank measuring 12 x 12.  There was a gap of 45 1/2 in. between the plank and the front uprights.  Employee(s) were exposed to a fall hazard.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation.  Duhon Blvd., Amelia, LA:  On or about July 24, 2013, the employer failed to ensure that the scaffold poles rested on base plates and mud sills.  Eleven (11) scaffold poles were missing base plates and were resting on stacked pieces of wood measuring 6" x13" and 11" x 14".

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  Duhon Blvd., Amerlia, LA:  On or about July 24, 2013, the employer did not ensure that employee(s) used a ladder to access the upper level of the scaffold.  Employee(s) were observed climbing the scaffold crossbraces to gain access to the working platform.  Employee(s) were exposed to a fall hazard.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Duhon Blvd., Amelia, LA:  On or about July 24, 2013, the employer did not ensure that the working platform of the scaffold was adequately guarded.  Employee(s) were working approximately 12 ft. high without a guardrail system in place.  Employee(s) were exposed to a fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.
