29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a) Two story residential dwelling located at corner of West Washington Avenue and Aetna Street, Connellsville, Pennsylvania 15425: Observed on or about July 24, 2013, employees engaged in roofing activities on two story residential dwelling were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. The slope of the roof varied from 3 in 12 (vertical to horizontal) along the outer edges to 7 in 12 (vertical to horizontal) toward the center. The employees were exposed to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards and the procedures to be followed to reduce or eliminate these hazards: a) Two story residential dwelling located at corner of West Washington Avenue and Aetna Street, Connellsville, Pennsylvania 15425: Observed on or about July 24, 2013, employees engaged in roofing activities on two story residential dwelling were not provided with fall protection training. The employees were exposed to a fall hazard of approximately 20 feet to the ground below.
