Machine Shop Area Level 1- On or about July 23, 2013 employees were required to perform work on part revolution mechanical power press machine that is not adequately guarded. Make of machine: USI, Cleaning Press, Model number of machine: 60P730, Serial number of machine: 57-9232. Left side of plexiglass barrier guard that is currently installed does not totally enclose entire point of operation hazard area. Thereby exposing employees to cuts, lacerations, caught in, and amputation hazard(s).

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.  a. Shop Area Level 1- On or about July 24, 2013, employees are required to perform tasks on a part revolution power press machine that was not properly guarded to prevent employees from reaching into in-running nip points. Make of machine: USI Cleaning Press, Model number of machine: 60P730, Serial number of machine: 57-9232. Thereby exposing them to in-running nip points, caught by, and amputation hazard(s).
