29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)  On or about June 18, 2020, the employer failed to ensure an energy control program consisting of an energy control procedure, employee training and periodic inspections was established for employees performing service and maintenance on machines including, but not limited to Clearing Torc Pac 60 Punch Press, Serial Number 57-9232, and Clearing Torc Pac 45 Punch Press, Serial Number 85-1137-77.  Employees were thereby exposed to hazards associated with rotating and moving parts.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:      (a) On or about June 18, 2020, in the Print Shop, the employer failed to provide training to each authorized employee performing service and/or maintenance on the Torc Pac 60 Punch Press, Serial Number 57-9233, to ensure the purpose and function of the energy control program are understood and the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees, thereby exposing employees to crush and amputation injuries.

29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):    a) Press Room - On or about June 15, 2020, the employer failed to provide and ensure the usage of point of operation guards or properly applied point of operation devices on the Clearing Torc Pac 45 Punch Press machine, Serial Number 53-1560, when an employee placed his left hand in the point of operation and accidentally pressed the foot pedal, sustaining a crushed left hand, three (3) crushed fingers, and a partial index finger amputation.
