29 CFR 1910.146(e)(1) Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: Location: #49 Stoneleigh Towers, Olivette, MO 63132 On or about July 24, 2013, the employer failed to ensure safe confined space entry conditions when employees entered the downstream manhole (17L2- 149S) to retrieve the packer without using a retrieval harness. A confined space entry permit had not been filled out for this day. Among other methods, recognized and feasible means of abatement to correct the hazard include, but are not limited to: (1) Implementation of the Confined Space Program outlined in the company's Safety Manual. (2) Implementation of the elements required by the American National Standards Institute (ANSI/American Society of Safety Engineers (ASSE) Standard Z117.1-2009, Safety Requirements for Confined Spaces. (3) Implementation of the elements required by the American National Standards Institute (ANSI/American Society of Safety Engineers (ASSE) Standard Z359.4, Safety Requirements for Assisted-Rescue and Self-Rescue Systems, Subsystems and Components.
