29 CFR 1926.351(b)(2): Welding cable(s) in use were not free from improper repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected. a) Ready Shack, Hangar 3: On or about 23 July 2013 and times prior thereto, an electrical welding cord had a damaged cord approximately eight inches from the stinger exposing wires. This condition exposed employees and electrical shock and burn hazard. b) Roof Top working area, Hangar 3: On or about 23 July 2013 and times prior thereto, two electrical welding cords had damaged cords. One cord was plugged into a Miller Maxstar 150S with a damaged cord approximately nine foot, nine inches from the stinger. The second cord was plugged into another Miller Maxstar 150S with a damaged cord approximately five feet from the stinger. These conditions exposed employees to an electrical shock and burn hazard.

Roof Top working area, Hangar 3: On or about 23 July 2013 and times prior thereto, an electrical junction box connected to a Miller Maxstar 150S missing three screw in plugs. This condition exposed employees to an electrical shock and burn hazard.

Configuration of the equipment would allow employee to fall more than three foot and from a height of approximately 40 feet. This condition exposed employees to a fall hazard.

Roof Top working area, Hangar 3: On or about 23 July 2013 and times prior thereto, safety measures were not in place for employees working on edge of hangar. The fall arrest system was connected to a self-tracking rope grab with integral 3' shock absorbing lanyard connected. Configuration of the equipment would allow employee to fall more than three foot and from a height of approximately 40 feet. This condition exposed employees to a fall hazard.
