"The Gateway to Weehawken", 1721-1723 Willow Avenue, Weehawken, NJ - 5th floor, west side interior of building.      A Werner A-frame stepladder was used in a manner other than the purpose for which it was designed.  Employee used an A-frame ladder in the closed position to perform his work.    Violation observed on or about July 24, 2013

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a. "The Gateway to Weehawken", 1721-1723 Willow Avenue, Weehawken, NJ - 5th floor, west side interior of building.  An employee was observed working from an A-frame stepladder that had broken steps.  The broken ladder was not inspected and taken out of service.  Violation observed on or about July 24, 2013

29 CFR 1926.501(b)(2)(ii): Each employee on a walking/working surface 6 feet (1.8 m) or more above a lower level where leading edges are under construction, but who was not engaged in the leading edge work, was not protected from falling by a guardrail system, safety net system, or personal fall arrest system.  a. "The Gateway to Weehawken", 1721-1723 Willow Avenue, Weehawken, NJ - southeast corner, 6th floor    Employees engaged in poured in place concrete construction work were not protected from falls by use of a guardrail system, safety net system, or personal fall arrest system.    Violation observed on or about July 23, 2013  b. "The Gateway to Weehawken", 1721-1723 Willow Avenue, Weehawken, NJ - northwest corner, 6th floor    Employees engaged in poured in place concrete construction work were not protected from falls by use of a guardrail system, safety net system, or personal fall arrest system.    Violation observed on or about July 24, 2013  River Front Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR�1926.501(b)(5), which was contained in OSHA inspection number 901153, citation number 1, item number 1, and was affirmed as a final order on May 14, 2013 with respect to a workplace located at 1800 Willow Avenue, Weehawken, NJ, 07086.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:     a. "The Gateway to Weehawken", 1721-1723 Willow Avenue, Weehawken, NJ - 5th floor.     Reinforced steel was not capped.  Employees performing concrete finishing activities at the 5th floor were not protected from impalement hazards of uncapped reinforced steel (rebar).     Violation observed on or about July 24, 2013        b. "The Gateway to Weehawken", 1721-1723 Willow Avenue, Weehawken, NJ  6th Floor.     Reinforced steel was not capped.  Employees performing decking and ironwork activities at the 6th floor were not protected from impalement hazards of uncapped reinforced steel (rebar).     Violation observed on or about July 24, 2013        River Front Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR�1926.701(b), which was contained in OSHA inspection number 901153, citation number 1, item number 2, and was affirmed as a final order on May 14, 2013 with respect to a workplace located at 1800 Willow Avenue, Weehawken, NJ, 07086.
