29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Location: 195 Binney Street, Cambridge, MA On or about 7/25/13, workers were exposed to fall hazards of up to 20-feet in the following instances; 1. A worker window washing from a boatswains chair was tied off to the frame of the HVAC rooftop unit. 2. Workers walking and working on top of the roof-top of the building within 15-feet of the edge of the roof. 3. Workers accessing a vertical ladder to access a lower roof.
