29 CFR 1910.147(c)(7)(i)(B): Affected employees were not instructed in the purpose and use of the energy control procedure:  a) Plant - On or about 07/24/13 employees applied lockout/tagout for maintenance and repairs of equipment including (but not limited to) the green TSInc foam grinder, the blue Fabtec foam grinder, and Molding Machines 1, 2, 3, and 4.   Affected employees were not trained on the control of hazardous energy and lockout/tagout.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective.  a) Plant, Molding Machine #4 -  On or about 07/24/13 the yellow extension cord which powered the Newpoint relocatable power tap did not have a ground pin in the male plug.  b) Plant, Molding Machine #4 - On or about 07/24/13 the 26 in. diameter air circulator powered by the Newpoint relocatable power tap did not have a ground pin in the male plug.  c) Plant, Molding Machine #4, aisle - On or about 07/24/13 the 25 in. Utilitech air circulator (powered by the grey relocatable power tap) did not have a ground pin in the male plug.   d) Plant, Molding Machine #4, aisle - On or about 07/24/13 the 44 in. diameter galvanized air circulator (powered by the grey relocatable power tap) did not have a ground pin in the male plug.  e) Plant, Molding Machine #4, aisle - On or about 07/24/13 the grey relocatable power tap (120V, 15A) did not have a ground pin in the male plug. 29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    a) Plant, Molding Machine #4  -  On or about 07/24/13 the yellow extension cord which powered the Newpoint relocatable power tap had a loose strain relief device at the female plug.    b) Plant, Molding Machine #4, aisle -  On or about 07/24/13 the 25 in. diameter Utilitech air circulator had a loose strain relief device at the male plug. The outer jacket was withdrawn and the internal conductors and terminals were subjected to strain.    c) Plant, Garage Door Machine - On or about 07/24/13 the flexible cord for the light in the front of the machine had a loose strain relief device at the male plug.  The outer jacket was withdrawn and the internal conductors and terminals were subjected to strain.    ACH Foam Technologies, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(2)(iii), which was contained in OSHA inspection number 314023417, citation number 1, item number 4, and was affirmed as a final order on  06/02/10, with respect to a workplace located at 4001 Kaw Drive, Kansas City, KS 66102.
