29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: 2210 S. Tanger Blvd., Gonzales, LA: On or about May 15, 2013, employer failed to ensure that the scaffold work platform was fully planked to prevent employees from falling in the approximate three feet gap between the one installed plank and the front guardrails. An employee was observed working from the outrigger (one plank) without any planking between the front uprights and the guardrail supports. The employee was exposed to a fall of approximately 12 feet.

Tanger Blvd., Gonzales, LA: On or about May 15, 2013, the employer did not ensure that the scaffold work platform was a minimum of 18 inches wide. An employee was observed working from the outrigger on one plank approximately 12 inches wide. The employee was exposed to a fall hazard of approximately 12 feet.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. 2210 S. Tanger Blvd., Gonzales, LA: On or about May 15, 2013, the employer failed to ensure that employees working from a fabricated frame scaffold two bucks high were provided with an appropriate means of access and egress. Employees were using the scaffold crossbraces to gain access to the upper levels, exposing themselves to a fall hazard of approximately 12 feet.
