29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  (a). HCA Midwest Division: The employer failed to assess the workplace to determine the occupational noise levels present in the workplace. Employees were exposed to high noise levels while working with lawn mowers, weed eaters and blowers.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. [63 FR 33450, June 18, 1998]  (A) At HCA Midwest Division:  The employer failed to maintain and repair an existing eye wash station that did not have  sufficient water flow to drench both eyes in case of an emergency. Employees were exposed to corrosive chemicals such as, but not limited to: FX2 Herbicide.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a). HCA Midwest Division: The employer failed to develop and implemement a written hazard communication program. Employees were exposed to hazardous chemicals while mixing, handling, spreading and spraying herbicides, insectacides, fungicides and fertilizers during landscaping activities.

29 CFR 1910.1200(f)(9): The employer did not ensure that labels or other forms of warning were legible, in English, and prominently displayed on the container, or readily available in the work area throughout each work shift:  (a). HCA Midwest Diviasion: Employer was allowing employees to utilize unlabeled containers from hazardous chemicals to water plants, the containers are also stored near drinking water. Employees are using unlebled containers to store and mix hazardous chemicals throughout the shop and in the transport trucks to outside work sites not in continuous use.
