29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a). Inside the two story single family residence no guardrail was installed to protect the employees working on the second level where it was opened to the floor below exposing them to a 12-foot fall hazard as observed on or about July 26, 2013.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On the roof of the two story single family residence on Lot 1128 where employees were installing roof decking, no fall protection was being used to protect the three employees from an 18-foot fall hazard from the eave to the ground below as observed on or about July 26, 2013.

29 CFR 1926.1053(b)(1):  Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.  (a) At Lot 1138 of 1204 Legg Drive in the D.R. Horton development, the 20-foot multi-purpose extension ladder used by employees to gain access to the interior second floor landing of the single family residence was not positioned with the side rails 3 feet above the landing exposing them to a 15-foot fall hazard as observed on or about July 26, 2013.

29 CFR 1926.503(a)(2):  The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii).  (a).  The employer did not assure that each employee installing roof sheeting on the roof of the two story single family residence at Lot-1138 were trained on fall hazards and the protective measures to implement while working at heights greater than 10 feet.
