29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  a. Scaffold east area - ladder used for access to the welded frame scaffold was partially taken down and did not extend for completed access to the second level; employees used an endframe for access and were exposed up to a 13' 9" fall hazard.  Violation observed on or about 12/18/13.

Scaffold east and south walls - single cross bracing sections were used on open sides of platforms as both a mid and top rail during masonry operations and exposed employee(s) up to a 13' 9" fall hazard on a welded frame scaffold.  Violation observed on or about 12/18/13.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  a. Mixing area - employees that used 3M 8210 N95 dust masks during masonry operations were not provided the information contained in Appendix D of 29 CFR 1910.134.  Violation observed on or about 12/18/13.
