29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: In the Maintenance Shop Area above the Maintenance Office: The employer failed to provide a standard railing with toeboard to protect employees from hazards caused by items stored on top of the Maintenance Office from falling off and striking the employees. As well as potential fall issues caused if employees were to get on the Maintenance Office to retrieve items stored in the center of the floor.

The employer did not ensure that effective chip guarding and or personal protective equipment was used by employees as well as the air pressure was not reduced to 30 p.s.i.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate. In the Maintenance Shop Area above the Maintenance Office: The employer permitted the storage of materials, supplies and miscellaneous objects on the floor of the roof of the office. The employer failed to ensure that the approved floor load limit was marked in a conspicuous place.
