Maintenance employees performed tasks such as, but not limited to, changing bearings, switches, drives, photo eyes, and other equipment on industrial milk filling equipment. Hazardous energy existed in the form of 110/220V electricity, 480V electricity in the electrical room, compressed air, and mechanically moving parts.  No lockout tagout program was in place for any of the equipment at the establishment.
