29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) On or about 10/12/2018, at the St Armands Baking Co. facility, employees of Dakin Dairy engaged in maintenance work on a 480-volt Sani-Tech compactor. Neither St Armands Baking Co. nor Dakin Dairy performed lockout-tagout procedures for the compactor at its energy source, exposing employees of Dakin Dairy to caught in between/amputation hazards.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4): On or about 10/12/2018, at the St Armands Baking Co. facility, an employee of Dakin Dairy used a portable grinder that was not guarded to cut a bolt off to gain access to a drive chain for a compactor.
