The assessment of appropriate personal protective equipment (PPE) for employees to utilize when working from unguarded work platforms was not conducted nor was fall protection equipment available for employees to use.:    a)  Welders were exposed to an approximate 31 foot fall hazard to the ground below as they descended from the conveyor catwalk down a 7 foot fixed ladder to the 12 inch wide grated ladder landing and bin platform as a 6 foot section of midrail was not installed;    b)  Welders were exposed to an approximate 8 foot fall hazard into the gravel bin as they stood on the unprotected 12 inch wide grated platform to position a portable extension ladder for bin access and egress.
