29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    Location:  201 Marshall Road, Valley Park, MO  63088    Valley Park Welding, LLC failed to conduct a workplace hazard assessment of the jobsite located at 201 Marshall Road, Valley Park, MO  63088.  The assessment of appropriate personal protective equipment (PPE) for employees to utilize when working from unguarded work platforms was not conducted nor was fall protection equipment available for employees to use.:    a)  Welders were exposed to an approximate 31 foot fall hazard to the ground below as they descended from the conveyor catwalk down a 7 foot fixed ladder to the 12 inch wide grated ladder landing and bin platform as a 6 foot section of midrail was not installed;    b)  Welders were exposed to an approximate 8 foot fall hazard into the gravel bin as they stood on the unprotected 12 inch wide grated platform to position a portable extension ladder for bin access and egress.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:  Location: 201 Marshall Road, Valley Park, MO  63088    Valley Park Welding failed to select and have the workers use appropriate fall protection to protect the workers from the fall hazards.  The workers were allowed to work from an unguarded platform without fall protection equipment to protect them from a fall hazard into a permit required confined space.  Additionally, employees were exposed to an approximate 31 fall hazard from the elevated gravel bin to the ground below as an approximate six foot section of midrail was missing at the ladder landing.
