29 CFR 1910.134(c)(1) :A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) On or about January 28, 2022, where the were welders were issued and were required to use tight-fitting respirators for protection against welding fumes, the employer did not develop and implemented a written respiratory protection program. b) Medical evaluation of the worker's ability to wear respirators was not conducted prior to allowing the workers to use respirators. c) Fit testing of the workers to with the respirators they used was not conducted prior to allowing workers to use the respirators.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 28, 2022, in the repair shop where a wellington bandsaw was used on routine bases, part of the saw was not guarded to protect the workers.
