OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the overhead hoist or rail/beam system fail:  '  a)  The following bridge rail/beam had no rated load markings and no end stops:        1.  Shipping Area, on or about 7/31/13:  There were no rated load capacity markings on the rail/beam for a Coffing 1/2 ton overhead crane and no rail end stops provided.        2.  Machining Area by American lathe, on or about 7/31/13:  There were no rated load capacity markings on the rail/beam for a CM 1/2 ton overhead hoist and no rail end stops provided and on a jib crane, with a CM 1/4 ton overhead hoist, there were no rated load capacity markings on the jib.    Among other methods, feasible and acceptable abatement methods to correct these hazards are to:        1.  Mark the proper rated load capacities on the rail/beam/jib in accordance with ASME B30.11-2010, Section 11-1.1.1.        2.  Install end stops on the bridge rails to prevent the hoist from damage in accordance with ASME B30.11-2010, Section 11-1.3.1(g).      b)  The following overhead hoists had a broken hook safety latch and the upper limit switches were not functioning.        1.  Shipping Area, on or about 7/31/13:  A Coffing 1/2 ton overhead hoist had a broken lifting hook safety latch.        2.  Machining Area, by the American lathe, on or about 7/31/13:  A CM 1/2 ton overhead hoist did not have a functioning upper limit switch.        3.  Stock Room, on or about 7/31/13:  A CM 1/2 ton overhead hoist did not have a functioning upper limit switch.    Among other methods, feasible and acceptable abatement method to correct these hazards are to:        1.  Replace the safety latch on the hook in accordance with ASME B30.16-2010, Section 16-1.2.9.        2.  Repair/adjust the upper limit switch on the hoist in accordance with ASME B30.16.2010, Section 16-1.2.14.    Abatement certification must be submitted for these items.
