29 CFR 1910.22(d)(1): Walking-working surfaces were not inspected, regularly, and maintained in a safe condition:  a)  Shipping Dept., on or about 2/13/18:  Concrete floor near the UPS bench was broken up in two areas creating a tripping hazard.  b)  Final Inspection, on or about 2/13/18:  Outdoor exit route was not smooth and solid in that there was an uncovered 9 inch plastic drainage pipe in the ground along the exit path just outside the exit door.  Abatement certification must be submitted for this item.

29 CFR 1910.25(b)(3): The employer did not ensure that the stairs had uniform riser heights and tread depths between the landings:    a)  CNC 2, on or about 2/13/18:  Stairways to three exits at the NAK #1, NAK #6 and MAT 760 machining centers were not uniform throughout in that the steps measured 8 inches, 9 inches and 7 inches and the threshold plate at each of the doorways stuck out at the top riser exposing employees to a tripping hazard.
