Rotating notched chuck 61 inches from the ground, near the packing area, was not provided with safeguarding. B. On or about August 1, 2013, Loading Dock: The Northstar Disposal compactor was not provided with a positive pressure switch or an interlock.

Kitchen: A flexible extension cord was used to power the Technoclipper TC 1590 E-N control panel. C. Stuffing Room: A flexible extension cord was used to recharge the Prime Mover pallet jack, Model PMX45, S/N PMX4534301026.
