On or about October 31, 2014:  In the following locations, water was allowed to accumulate on the workroom floor:  1.  Boiler Room, near a 240V outlet on the aisle side of the Bigelow boiler. 2.  Boiler Room, at the back of the Hartford Boiler, near a wall-mounted 2-outlet receptacle with a missing knockout. 3.  Compressor Room, near live electrical components on Vilter Screw Compressor #7 and the open door of the Benshaw 208V service disconnect #6.

On or about October 31, 2014, Boiler Room:  Access to 208V electrical panels was blocked by the placement of a 26" diameter pedestal fan and a floor-mounted Gardner-Denver compressor.  B.  Tool Room/Parts Storage Room:  A 208V electrical panel was blocked by the storage of wooden boards and a Schumacher 10/40 amp battery charger.
