29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    An employee engaged in maintenance activities throughout the workplace was exposed to electrical shock, struck-by and caught-between hazards while performing maintenance on the printing presses and ovens in that a comprehensive energy control program had not been developed and implemented.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:    Employees engaged in production activities in the Production Area were exposed to electrical shock hazards in that the following electrical sources were not free from recognized hazards:      a) Employees engaged in printing activities at press #G3 in the Print and Stack Department were exposed to electrical shock hazards in that the flexible cord supplying power to the press had been damaged and covered using white tape.     b)  Employees engaged in production activities at the Precision Oven in the Production Area were exposed to electrical hazards in that the flexible cord supplying power to the oven had been spliced together using wire nuts and was not in conduit.

29 CFR 1910.303(g)(2)(i)(B): Except as elsewhere required or permitted by this standard, live parts of electrical equipment operating at 50 volts or more was not guarded by suitable permanent, substantial partitions or screens so arranged so that only qualified persons would have access to the space within reach of the live parts. Any openings in such partitions or screens were not so sized and located that persons were not likely to come into accidental contact with the live parts or to bring conducting objects into contact with them:    Employees engaged in production activities throughout the Production Area were exposed to electrical shock hazards in that the following live electrical sources were not protected:    a)  The terminal compartment panel at the Lehr 2 oven was missing, the location was marked as high voltage, 480 volt.   b)  Two junction boxes at the Precision oven were missing covers.  c)  A thermostat control at the Precision oven was missing a cover.  d)  The electrical cover at press #G3 was missing.
