29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program: a. Southern Forest Industries, Inc. - Sawmill Area - An employee operating the Bander was exposed to a noise level of 92.5 dBA, which exceeds the action level of 85 dBA, during the 8-hr representative sampling period on August 7, 2013. The employer failed to provide employees annual training to ensure employees required to wear hearing protection were familiar with the hazards of noise exposure. b. Southern Forest Industries, Inc. - Sawmill Area - An employee operating the Sorter was exposed to a noise level of 88.5 dBA, which exceeds the action level of 85 dBA, during the 8-hr representative sampling period on August 7, 2013. The employer failed to provide employees annual training to ensure employees required to wear hearing protection were familiar with the hazards of noise exposure.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein: On or about July 31, 2013: Employees in the sawmill operating machinery such as but not limited to: banders, sorters, stackers, and other equipment, are exposed to contact hazards from oil used for equipment maintenance. The employer did not ensure all containers of oil were labeled with their contents as required.
