29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:       On or about July 31, 2013 at worksite 2034 Conneticut in Joplin, Missouri- Employees were exposed to electrical shock and burn hazards in that the flexible cord was not connected to a device or fitting that provided strain relief or prevented pull from being directly transmitted to joints or screws.     Abatement certification for this violation is not required.

29 CFR 1926.501(b)(11):   Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a)  On July 31, 2013, at  worksite 2034 Conneticut, Joplin, MO., Five employees engaged in roof sheathing work on a steep pitched roof were exposed to serious fall hazards, in that no Fall Protection Equipment or Fall Protection Systems were in use. Employees were exposed to serious injuries from fall hazards ranging from approximately 15 - 25 feet.       Abatement certification for this violation is not required.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about July 31, 2013, at a job site located at 2034 Conneticut in Joplin, MO, workers were exposed to serious fall hazards approximately 15 - 20 feet and provided no training.
