(a) Front side, new residential construction located at 2686 Telfare Drive, Smyrna, GA 30080: Employee using an elevated job-made scaffold platform resting on the forks of a Lull (Model 644E-42) Telescopic Forklift installing 1/2-inch OSB plywood at heights of approximately 18-feet, 8-inches was not protected from falls to the ground level. Employee using this scaffold platform was not provided with guardrails or a personal fall protection system.
