29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    a.) The Quikrete Companies located at 873 Western Toledo, OH 43609; On or about August 6, 2013, the employer did not assure pallets of bags of blacktop were securely stacked. Employees were exposed to struck-by and crush hazards while accessing the black top and Quikrete storage area.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:     a.       The Quikrete Companies located at 873 Western Toledo, OH 43609; On or about August 6, 2013, the employer did not assure employees used electrical safety related work practices when accessing the 230/460V electrical panel of the cement auger to verify zero energy, in that proper Personal Protective Equipment (PPE) was not worn. Employees were exposed to electrical shock and arc flash hazards from the 230/460V equipment.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:     a.)       The Quikrete Companies located at 873 Western Toledo, OH 43609; On or about August 6, 2013, the employer did not assure employees wore personal protective equipment, including but not limited to, insulated gloves with leather gloves, arc rated clothing, face shield and hard hat while verifying zero energy. Employees are exposed to electrical and shock hazards when changing a fuse in a 230/460V electrical cabinet for the cement auger.
