On or about August 13, 2013, an operator working in the lamination process area was exposed to toluene-2,6-diisocyanate (TDI). Exposures occurred via inhalation at concentrations of 0.08 milligrams per cubic meter.

Respiratory protection and a respirator program was not developed and implemented for this facility.

On or about August 13, 2013, an operator working in the lamination process area was exposed to toluene-2,4-diisocyanate(TDI). Exposures occurred via inhalation at an average concentration of 0.32 milligrams per cubic meter. This concentration was 2.25 times the permissible ceiling limit of 0.14 milligrams per cubic meter.

Exposures occurred via inhalation at an average concentration of 0.32 milligrams per cubic meter. This concentration was 2.25 times the permissible ceiling limit of 0.14 milligrams per cubic meter. Engineering controls did not effectively reduce employee exposure to required limits.

Chemicals hazards such as but not limited to those caused by hydrogen cyanide, 2,4-toluene diisocyanate, hexamethylene diisocyanate and 2,6-toluene diisocyanate.
