29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: At this location, employees who worked in the controller station of the Lamination Machine, were required by the employer, to wear a tight-fitting, half-face respirator to protect them from exposures to isocyanates. The employer allowed the employees to wear a respirator with facial hair that came between the sealing surface of the face piece.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. At this location, maintenance employees were exposed to electrical hazards and struck-by hazards, when they were performing maintenance on the Taiseitec Lamination Machine. The employer didn't establish a specific Lock out/Tag out procedure for the lamination machine.
