Medical Center in Leeds, MA: On or about Sep 4, 2013, in and around rooms 1292 - 1299 of building #1 and in and around rooms 127 and 130 of building #2, carpeting was observed to be buckled and raised exposing employees to a fall on same level, trip hazard which is not in accordance with this subpart.

The employer failed to follow the requirements of this subpart.
