Boiler Room, 421 North Main St, Leeds, MA On 10/9/14, the key to unlock ockout devices that were affixed on 9/13/14 to the #1 boiler was found located in an unlocked group lock box and available to anyone.

29 CFR 1960.28(d)(3): The Agency inspections were not conducted within 24 hours for employee reports of imminent danger conditions, within three working days for potentially serious conditions, and within 20 working days for other than serious safety and health conditions: Mail Room, 421 North Main St, Leeds, MA As of 11/19/14, the employer did not perform an inspection of a reported potential noise hazard from mail room machinery within the 3 working day requirement. A Union reported the noise issue in March 2014 and OSHA measured the noise at 91.6 decibels on 10/9/14. On 11/19/14, OSHA found that an inspection of the potential noise hazard had still not been completed and the machines were in use.
