While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings: Trap door, Card 4, first floor, S&D Spinning Mill, Inc., 190 West Main Street, Millbury, Massachusetts; The trap door on the collection pit was not maintained and was degraded, leaving a triangular hole, approximately 6 inches x 6.5 inches x 8.8 inches, that presented a tripping hazard.
