On or about 7/30/2013, Sea Chest/Ace Hardware, 3806 Crown Bay, St Thomas VI - warehouse area:  Power industrial truck were used on a daily basis to transport warehouse materials during loading/unloading operations.  The employer did not certify that each operator had been trained and evaluated.    Note:   Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

On or about 7/30/2013, Sea Chest/Ace Hardware, 3806 Crown Bay, St Thomas VI  - Building second floor area:  Employees were using electrical power tools connected to an electrical receptacle lacking a ground fault circuit interrupter (GFCI).       NOTE:  Certification and documentation of abatement must be submitted for this citation item as required by 29. CFR 1903.19(c) and (d), respectively.
