29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.     Construction Site, outside perimeter, near electrical panel board -   Noted on or about 8/8/13 and at times prior thereto, ground fault circuit interrupter protection or an assured equipment grounding conductor program was not used when a 110 VAC right angle grinder was powered through an extension cord. The system did not trip when tested.  Employees were exposed to the hazards of contacting electricity.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:     Construction Site, stairs leading to second level - Noted on or about 8/8/13 and at times prior thereto, stairs with 12 risers (36"W, 94.5" fall hazard from top riser to ground) were not equipped with a handrail.

29 CFR 1926.51(f)(1): The employer shall provide adequate washing facilities for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be in near proximity to the worksite and shall be so equipped as to enable employees to remove such substances.   Construction Site - Noted on or about 8/8/13 and at times prior thereto,  adequate washing facilities were not provided for employees observed applying Behr Ultra Pure White 1750 paint on interior walls on the ground level.  Soap and hand towels were not provided.  Employees did not wear gloves and were observed with paint on their fingers and hands.
