29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:   (a)  On or about 8/9/13 - Blue Creek Lumber mill areas, employees are assigned to work in areas such as the head saw and edger positions and are exposed to noise levels exceeding 85dBA:   On or about 8/19/13 - The Off Bearer (headsaw) position was exposed to continuous noise at 145 % of the allowable 8-hour time-weighted average sound level of 85 dBA or an equivalent level of 99.3 dBA.  The sampling was performed for 193 minutes during one shift.   On or about 8/19/13 - The edger position was exposed to continuous noise at 65.2% of the allowable 8-hour time-weighted average sound level of 85dBA or an equivalent level of 93.9 dBA.  The sampling was performed for 183 minutes during one shift.
