29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: (a) 4109 Iron Horse Trail: On or about August 2, 2013, and at times prior, employees were exposed to a fall of approximately twenty-five feet to the ground below installing asphalt shingles on a 6:12 pitch steep sloped roof. (b) 4117 Iron Horse Trail: On or about August 2, 2013, and at times prior, employees were exposed to a fall of approximately twenty-three feet to the ground below installing asphalt shingles on a 6:12 pitch steep sloped roof.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Construction Site: On or about August 2, 2013, and at times prior, employees were exposed to a fall of approximately twenty feet to the concrete below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
