29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Construction site: On May 20, 2015 and at time prior, employees were exposed to head injuries from falling items such as but not limited to, hand tools and roofing material while walking and working below roofing activities.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: (a) Construction site: On May 20, 2015 and at time prior, employees were exposed to a fall hazard of approximately 24 feet to the ground below while installing asphalt shingles on a residential building with a 6/12 pitch roof. Great White Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 927548, citation number 1, item number 1 and was affirmed as a final order on December 5, 2013, with respect to a workplace located at 4109/4117 Iron Horse Trail, Billings, MT 59106.
