29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:       South San Antonio Yard:  On or about August 7, 2013, employees were exposed to slip, trip, and fall hazards from walking on silica frac sand, which leaks from the bottom of rail cars, while lacing hoses to operate the air brakes.

29 CFR 1910.151(b): First aid supplies approved by a consulting physician were not readily available:       a)  East San Antonio Yard:  On or about August 7, 2013 employees were potentially exposed to cuts or lacerations while performing switchman duties on the rail yard and there was no first aid supplies available in the employees work area.       b)  South San Antonio Yard:  On or about August 7, 2013 employees were potentially exposed to cuts or lacerations while performing switchman duties on the rail yard and there was no first aid supplies available in the employees work area.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    East San Antonio Yard:  On or about August 8, 2013, a portable Ansul Red line fire extinguisher was located directly on the ground under a bench in the employee shade area and was not labeled or identified.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:       South San Antonio Yard:  On or about August 7, 2013, employees were potentially exposed to silica, and were not trained on the health hazards of silica which was accidently released from the rail cars that are removed from a line and built up to transport the rail cars to further destinations.
