29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees.    The employer does not ensure that lockout or tagout devices were affixed to each energy isolating device by authorized employees.                 a) This violation occurred on or about August 12, 2013, in Building 1 where operators of a Hyd-Mech Vertical Band Saw were exposed to struck-by and caught-in hazards when performing setup and maintenance tasks without using locks to isolate energy sources.        b) This violation occurred on or about August 20, 2013, in Building 1 where mechanics were exposed to struck-by and electrical hazards when performing maintenance tasks on equipment such as but not limited to, an iron spot welder and a miller welder without using locks to isolate energy sources.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all authorized employees are affixing lockout or tagout devices to each energy isolating for the control of potentially hazardous energy when employees are engaged in Lockout and Tagout Procedures.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.     The employer does not provide suitable facilities for quick drenching or flushing for the eyes or body where employees are using corrosive chemicals.  This violation was observed on or about August 12, 2013, in mechanic shop where employees were exposed to chemical burn hazards when changing out,cleaning and filling batteries without an eyewash in the work area.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have access to facilities for quick drenching or flushing of the eyes or body.

This violation was observed on or about August 12, 2013, in Building 1 where employees were exposed to a struck-by hazard when operating a Hyd-Mech Vertical Saw without a guard on the front cutting blade.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guards are installed on all machines to protect employees from the point of operation.

This violation was observed on or about August 12, 2013, in building #1 where wires to supply power to HV #1 Automatic Coping Machine were not provided with a covered enclosure.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all junction boxes are provided with complete enclosure for the contained conductors or cables.
