The employer does not ensure that lockout or tagout devices were affixed to each energy isolating device by authorized employees. a) This violation occurred on or about August 12, 2013, in Building 1 where operators of a Hyd-Mech Vertical Band Saw were exposed to struck-by and caught-in hazards when performing setup and maintenance tasks without using locks to isolate energy sources. b) This violation occurred on or about August 20, 2013, in Building 1 where mechanics were exposed to struck-by and electrical hazards when performing maintenance tasks on equipment such as but not limited to, an iron spot welder and a miller welder without using locks to isolate energy sources. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all authorized employees are affixing lockout or tagout devices to each energy isolating for the control of potentially hazardous energy when employees are engaged in Lockout and Tagout Procedures.
