OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to the hazards of being struck by materials being handled with a hoist being operated with a hook whose latch did not effectively close the hook's throat:    (a) Insulating area: The latch on the hook of the Coffing 1/2 ton electric hoist, unit #1, was broken, did not effectively close the throat of the hook, and was not removed from service or moused until the latch was replaced or repaired, on or about 8-13-13.    ."

(b) Insulating area I5: The Rohs Relocatable Power Tap (power strip) that was in use powering a strobe and inspection camera was not installed in accordance with instructions from the manufacturer and included in the listing and labeling, UL 1363, in that is was not directly connected to a permanently installed branch circuit, on or about 8-13-13.  (c) CLX workbench: The three Relocatable Power Taps (power strips) that were in use powering a drill press and bandsaw were not installed in accordance with instructions from the manufacturer and included in the listing and labeling, UL 1363, in that they were series connected (daisy chained) to each other, and were not directly connected to a permanently installed branch circuit, on or about 8-13-13.
