29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  The employer did not ensure that each powered industrial operator had received training and a performance evaluation (driving test).  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:   Repair Shop Area.  Relocatable power taps (electrical power strips), which were not designed for use in industrial environments, were mounted on the walls and used to power the following equipment:   a) A microwave on an extension cord   b) An extension cord used for testing snow blower starters   c) A time clock on an extension cord.   There is no abatement certification or documentation required for this item.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and cables were used where attached to building surfaces:  Repair Shop Area.  A flexible cord (110v) used as an extension cord for a microwave, was affixed to a wall and plugged into a relocatable power tap mounted to the wall.  There is no abatement certification or documentation required for this item.

29 CFR 1910.37(a)(3): Exit routes were not free and/or unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route.   An exit route to the exit door at the back of the warehouse was temporarily obstructed by materials stored on pallets and empty cardboard boxes.  There is no abatement documentation or certification required for this item.

Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c)
