29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.     a) An employee on the ground without head protection was working below other employees  on the roof removing old shingles, exposed to struck-by hazards on or about 08/14/ 13.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    a) Employees on a pitched roof disconnected from their fall protection exposing them to fall hazards of approximately 24 feet, on or about 08/14/13.     b) The crew leader was on a pitched roof without fall protection exposing him to fall hazards of approximately 24 feet, on or about 08/14/13.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back.   a) Employees on a pitched roof would tie off from the front of their positioning device located on the front of their body, on or about 08/14/13.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.     a) Employees were using an extension ladder on the main house to access the roof which was secured at the top but no grasping device was present to aid in use, on or about 08/14/13.    b) Employees were using an extension ladder on the porch to access the roof which was not three feet over the edge, on or about 08/14/13.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
