29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    A) 324 Swinton Avenue, Bronx - NY: Employees were exposed to fall hazards of approximately 33 1/2 feet while conducting waterproofing work on the roof of a residential structure, without the use of harnesses or other fall protection devices,  on or about, October 25th, 2023.

29 CFR  1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 324 Swinton Avenue, Bronx - NY : Employees were exposed to fall hazards of approximately 33 1/2 feet while using an extension ladder to access a roof. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about,  October 25th, 2023.
