Mattocks Five, Inc. dba Albion Mill, grain bin area- 73 Canal Street, Albion, Pa, 16401, on or about August 14, 2013, and times prior thereto:  Company employees voluntarily used 3M brand Full facepiece air purifying respirators during grain shoveling operations.  Company employees were not provided with a medical evaluation prior to the use of the 3M brand (#6800) full facepiece air purifying respirators.
