29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels :     a. An employee operating press #12 was exposed to noise on or about October 30, 2013. The employee had a time weighted average of 89.5 dBA and a dose of 93.72%, which is above the action level for hearing conservation program implementation. The total sampling time was 463 minutes, with the remaining 17 minutes considered as zero exposure.       b. An employee operating press #3 was exposed to noise on or about October 30, 2013. The employee had a time weighted average of 89.7 dBA and a dose of 96.24%, which is above the action level for hearing conservation program implementation. The total sampling time was 461 minutes, with the remaining 19 minutes considered as zero exposure.    On or about August 15, 2013, audiograms were not provided on an annual basis for employees exposed to noise above the action level.     RAM Plastics Co was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.95(g)(5)(i), which was contained in OSHA inspection number 110726, citation number 1, item number 2b and was affirmed as a final order on February 16, 2012, with respect to a workplace located at 11401 South Avenue, North Lima, OH 44452.
