The condition exposed employees to fall hazards of up to 14.4 feet.

29 CFR 1910.27(b)(1)(ii): The distance between rungs exceeded 12 inches and were not uniform throughout the ladder:    (a)  Bottles Unlimited, Inc. at 5390 E. 39th Ave. Unit #3, Denver, CO 80207:  On and before August 14, 2013, the employer provided a fixed ladder for use in accessing the runway above the railroad cars, and the distances between the rungs varied as follows:  1)  The distance from the ground to the first rung was 38 inches;  2)  the distance between the next six rungs measured between 18 inches and 19 inches;  and 3)  The distance from the seventh rung to the runway was 21 inches. The condition exposed employees to a fall hazard of up to 14.4 feet.

29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material was not provided to permit easy handling of material without the operator placing a hand in the danger zone:    (a)Bottles Unlimited, Inc. at 5390 E. 39th Ave. Unit #3, Denver, CO 80207:  On and before August 14, 2013, the employer did not require the use of special hand tools for removing excess plastic from the trimmer for Extruder #3.  the condition exposed employees to amputation hazards.    Abatement note:  By depending only on the interlock to de-energize the trimmer, employees are exposed to amputation hazards.  Two additional steps are needed to safely clear the excess plastic around the trimmer:  1)  Press the red E-Stop button all of the time;  2)  Use extension tools all of the time.  Establish a clear expectation by defining the danger zone as any area behind the guards.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  Bottles Unlimited, Inc. at 5390 E. 39th Ave. Unit #3, Denver, CO 80207:  On and before August 14, 2013, the employer did utilize lockout tagout procedures when working on the BAE extruder machine.  The condition exposed employees to amputation hazards.  Abatement note:  Even though these machines may require power while the maintenance work is being performed, the machines should be locked out and tagged out when not in use and when not being worked on.  In this case, the BAE machine was last in use three weeks earlier, and was not being worked on at the time of the inspection.

The condition exposed employees to struck-by hazards.
