Examples of guarding methods are-barrier guards, two-hand controls, and electronic safety devices:    (a) Bottles Unlimited at 3950 E. 39th Ave Denver, CO 80207: On or before January 24th, 2015, the employer did not ensure that machine guarding was in place for the Hesta blow mold machine model HMD360E, serial number 11.5 2013 10676, to protect the operator and other employees in this machine area from the bottle top cutting blade.  Condition exposed employees to an amputation hazard by contact with the cutting blade.
