29 CFR 1910.132(e): Defective or damaged personal protective equipment was used. On or about August 13, 2013, and at times prior thereto, the employer did not ensure that the DBA Sala fall protection harness attachments were in proper working condition, in that the snap hook locking gate was not locking, exposing employees to a fall hazard up to approximately 142 feet.
